This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.

Because our centers are federally funded, we are required to offer a discount to patients whose income qualifies them to receive one. In order to be considered for this discount, the patient MUST present the following:
1.Family income amount
2.The number of people in the household
3.Verification of income, i.e., check stubs, copy of check, W2 form, statement from employer or source of income or authorization for the center to check income source.
Medicare, Medicaid, and other private insurances can also be presented and used as a means to obtain health care.
